Substrate placeholder — needs reviewPressure has increased on Food and Drug Administration Commissioner Dr. Marty Makary from multiple directions as the White House seeks to adjust its healthcare agenda before the November midterm elections. Makary has had several clashes with President Donald Trump and White House staff in recent weeks over various issues.
Inside reports have suggested he may be on his way out. Makary left a surgical position at Johns Hopkins University to join the Trump administration. He previously gained attention for his criticism of the former Biden administration’s handling of the COVID-19 pandemic.
Since his appointment, the FDA has been central to several health policy issues facing the administration. Tensions between Makary and Trump increased in recent days. Anonymous administration officials told NOTUS that the FDA chief is on thin ice. Health and Human Services Secretary Robert F.
Kennedy Jr. considered delegating major FDA responsibilities to other officials late last year, which would have reduced Makary to a figurehead role.
Trump promised during the 2024 campaign to support vaping. Makary had moved slowly on approving new flavored vape and nicotine products. The Wall Street Journal reported that Trump scolded Makary over the weekend for not acting quickly enough. Within hours of that report, the FDA announced approval of four new vape flavors, including two fruit flavors, blueberry and mango, for the first time.
The FDA said the products underwent rigorous scientific review. The agency added that age verification requirements and marketing rules would minimize use by those under 21. White House spokesman Kush Desai described Makary’s work at the FDA as trailblazing leadership.
Desai said the commissioner will continue to make evidence-based decisions that rectify previous missteps and are in the best interest of the American people. Desai added that the only factor guiding the Trump administration’s health policymaking is Gold Standard Science.
Wade. Anti-abortion advocates and members of Congress have criticized Makary for not restoring requirements that mifepristone be dispensed only during an in-person doctor visit. The FDA removed those in-person requirements in 2023. A 2025 insurance data study found nearly 11 percent of women experience complications following at-home abortions.
Makary and Kennedy had promised a comprehensive safety review of mifepristone. Under Makary’s leadership, the FDA approved a new generic version of the drug in October. SBA Pro-Life America president Marjorie Dannefelser renewed her call to remove Makary.
She stated that indifference is completely unacceptable to millions of pro-life voters expecting the administration to act to save lives. Polling commissioned by her group found that one-third of GOP voters report being less enthusiastic about supporting Republican candidates who do not support tightening restrictions on abortion pills.
Pharmaceutical and biotech industry leaders have raised issues with personnel and procedures at the FDA. They have clashed in particular with Vinay Prasad, whom Makary appointed as head of vaccines and gene therapies. Prasad was characterized by industry leaders as confrontational.
Prasad briefly left the agency last summer after disputes with other administration officials but returned following Makary’s intervention with the White House. The administration announced earlier this spring that Prasad would leave again, with his last day set for May 7.
During his tenure, he had disagreements over safety trial data for gene therapies targeting Huntington’s disease and declined to approve Moderna’s new flu vaccine. Tim Hunt, head of the Alliance for Regenerative Medicine, told Bloomberg that the biotech community wants to see Marty Makary be successful but is frustrated by the agency’s toxic methodological purity.
Industry leaders said unpredictability at the FDA has disrupted the flow of products to market.
