Zanardi won two gold medals in the Paralympics in London in 2012. He won the H4 time trial and H4 road race at London 2012 Paralympics. Zanardi was selected as the Italy team's flagbearer at the closing ceremony of the London 2012 Paralympics.

He won gold in the H5 time trial and relay at Rio 2016 Paralympics. In addition, Zanardi became a 12-time world champion in para-cycling.

Zanardi won the men's para-cycling race at the New York marathon in 2011. He returned to motorsport after his 2001 crash, winning four times for BMW in the World Touring Car Championship from 2005 to 2009. Bologna-born Zanardi raced for Jordan, Minardi, and Lotus in Formula 1 in the early 1990s.
