It was operated by a single undertrained agent who had never tested the equipment before the event. When a call finally came in about the rooftop, that agent searched online for the building's location.
Classified intelligence about a long-range threat to the candidate was not shared with the Pittsburgh field office or the agents planning security. One bystander died. The candidate was grazed by a bullet.
Six staffers received suspensions ranging from 10 days to six weeks. The Secret Service agreed with all seven recommendations in the report.
