Bebe King, aged six, Elsie Dot Stancombe, aged seven, and Alice da Silva Aguiar, aged nine, were killed by 17-year-old Axel Rudakubana at a Taylor Swift-themed dance class.
The attack occurred on 29 July 2024 in Southport.
The public inquiry described a merry-go-round of referrals, assessments, and hand-offs between agencies that failed to stop the attack. It found that no agency or multi-agency structure accepted responsibility for assessing and managing the grave risk posed by the attacker. The inquiry's Phase One report is 760 pages long.
The public inquiry stated that the attacker's autism was used as an excuse for his past conduct, leading no agency to understand the true danger he posed. Inquiry chairman Sir Adrian Fulford called for the end of a culture of agencies passing responsibility between each other or downgrading their own involvement in such cases.
Families Demand Disciplinary Action Against Agency Individuals Chris Walker, legal representative for the families of the three murdered girls, said he will name key individuals from five agencies unless suitable disciplinary measures are taken.
The five agencies are Prevent, Lancashire Police, Lancashire Social Services, CAMHS, and FCAMHS. CAMHS is the Child and Adolescent Mental Health Service, and FCAMHS is the Forensic Child and Adolescent Mental Health Service. Prevent is a Home Office initiative aimed at stopping individuals from becoming extremists and carrying out attacks.
Chris Walker said he knows the names of individuals from the five public bodies. He wrote to all five agencies several months ago demanding disciplinary proceedings against the individuals. Chris Walker said he will follow up with the five agencies now that the inquiry report is concluded to ensure disciplinary proceedings satisfy the families.
He said Prevent failed to implement the ideological consequences or understanding in their various gateways.
Phase Two to Examine Risk Management for Extreme Violence Phase Two of the Southport Inquiry will begin immediately and report back in Spring 2027.
Phase Two will focus on the adequacy of arrangements for identifying and managing the risk posed by individuals fixated with extreme violence. The inquiry's findings on agency responsibilities and risk assessment stem from the events of the 29 July 2024 attack.
