In the fourth inning, Lugo threw a full-count changeup that Nimmo hit up the middle. The ball deflected off Lugo's forehead, causing him to drop to the ground.
Lugo stood without assistance and spoke with teammates and the Royals' athletic training staff as he walked off the field.
