Canadian agencies interacted with OceanGate across 10 departments yet never coordinated oversight of the Titan. The Transportation Safety Board found critical information remained disconnected between departments.

OceanGate first contacted Canadian officials while the Titan was still under construction in Everett, Washington.
In May 2021, Fisheries and Oceans Canada planned to pay the company $25,000 to support deep-sea research during planned Titanic missions the following year. Global Affairs Canada later denied a research permit after OceanGate incorrectly stated that Fisheries and Oceans Canada would serve as its sponsor.
The Titan’s first attempt to reach the Titanic occurred the next month and ended when one of its titanium domes detached.
The support vessel Horizon Arctic returned to St. John’s, where Border Security Agency officers directed it to a secure area before passengers could leave. Armed agents boarded the ship and questioned passengers about Covid-19 measures and their involvement in the dives.
Passenger Gary Philbrick described the officers as extremely intimidating and said he could not leave the ship fast enough. The agents also asked why OceanGate lacked a research permit. David Concannon, a lawyer who had previously worked with the company, told them the Titan would dive only in international waters.
Concannon later said the agents showed zero interest in the submersible itself and focused solely on paperwork. Transport Canada determined the Titan formed part of the Horizon Arctic’s cargo and therefore fell outside inspection requirements for passenger vessels.
In July 2021, a Fisheries and Oceans Canada researcher joined a later mission and reported that the carbon-fiber hull had received no regulatory approval or certification and carried no insurance.
Those findings never reached Transport Canada’s marine safety team. Fisheries and Oceans Canada did not proceed with its planned funding. Between 2021 and 2022, the Titan completed successful dives to the Titanic and other sites inside Canadian waters while OceanGate dealt with ten federal agencies, including Parks Canada, the Department of National Defense, and the Royal Canadian Mounted Police.
None of those contacts reached the marine safety officials responsible for vessel oversight. TSB investigator Jason Melvin stated that marine oversight staff concentrated on the Canadian support vessel rather than the Titan. TSB testing of leftover carbon-fiber samples showed porosity, waviness between layers, and grinding that could have introduced defects.
The same tests indicated the material could fail after as few as 30 deep dives, though calculations based on OceanGate’s original specifications suggested a hull might withstand hundreds of millions of dives. The Titan imploded on its 24th mission while diving deeper than 1,000 meters, killing all five people aboard, including OceanGate chief pilot and CEO Stockton Rush.
The Transportation Safety Board recommended stronger oversight of high-risk vessels, better information sharing across departments, and mandatory international construction and safety standards for all human-occupied submersibles.
