A dozen former presidents and world leaders gathered at the World Health Organization emergency hub outside Nairobi last Thursday for a simulation of a novel bird flu outbreak. The scenario described health workers in eastern Chad reporting respiratory deaths and initial samples pointing to a new variant.

Chad's government hesitated to notify the WHO within the required 24 hours due to concerns over economic damage and stigma.

Zedillo, a former Mexican president, said governments need incentives and assurances that timely reporting will not lead to punitive measures such as travel bans. Zeid bin Ra'ad bin Zeid al-Hussein, a former UN high commissioner for human rights, called for stronger verification systems across health sectors.

Ellen Johnson Sirleaf, a former Liberian president, said weak healthcare infrastructure often prevents accurate detection and reporting rather than a lack of political will.

The session occurred one day before health officials announced an Ebola outbreak in Uganda and the Democratic Republic of the Congo that has killed at least 139 people. Participants also noted that negotiators missed a deadline this month to finalize a global pandemic treaty first announced in 2021.

Denis Mukwege, a Congolese gynaecological surgeon and Nobel laureate, said the exercise highlighted the need for rapid multilateral action because outbreaks can cross borders. Dr Mohamed Janabi, WHO's regional director for Africa, told the group that the organization recorded 146 emergency disease outbreaks in the region last year.
