The 30-year-old Pole, ranked world number 76, claimed his first ATP title after two hours and 25 minutes on grass. Majchrzak had already beaten world number four Felix Auger-Aliassime and world number eight Daniil Medvedev in earlier rounds.
Both players faced blustery conditions, but Majchrzak took more risks and finished the stronger of the two. He took the first set 6-3 after breaking de Minaur's opening service game and closing with a high backhand volley at the net. De Minaur responded by winning the second set 6-2, breaking twice and capitalizing on a run of unforced errors from the Pole.
In the deciding set, Majchrzak broke serve in the seventh game on a double fault. De Minaur broke back immediately, but the set moved into a tie-break at 6-6. Majchrzak led 3-0 after another double fault from de Minaur, then saved a comeback to reach championship point.
De Minaur double-faulted on match point, giving Majchrzak the 7-5 tie-break win. "This is a very emotional moment for me, I've been waiting for this moment all my career," Majchrzak said after receiving the trophy. "I played some of the best players in the world, so this one feels really good, it feels really special.
The event in 's-Hertogenbosch served as a Wimbledon tune-up on grass courts.
