A crash five kilometers from the line caught Norway's Torstein Traeen, who retained the yellow jersey despite crossing the line 14 seconds behind Kooij.
Tadej Pogacar, Jonas Vingegaard, Remco Evenepoel, Paul Seixas, Tom Pidcock and Adam Yates all finished in the same group. "After a couple of hard days here already we've had to wait for this day to get this first chance to sprint in the Tour - and to immediately win is unbelievable," Kooij said.
He added that the stage remained easy until the final kilometers, when he found the right wheel and took his chance.
Kooij joined the Decathlon CMA CGM squad late after illness disrupted his early season. He has previously won three stages at the Giro d'Italia. Traeen leads the general classification after stage five in 16 hours 32 minutes 7 seconds.
American Sean Quinn sits second, 28 seconds behind. Pogacar and Vingegaard share fourth place, seven minutes and 53 seconds down.
